Description of the Sinus Infection

A sinus infection is an acute or chronic disease which affects the integrity of the nasal sinuses. Sinuses are very important elements and their role is to protect the brain against impacts, to reduce the weight of the skull and to improve the resonance of the voice.

Sinusitis appears when the nasal membrane gets affected by pollutants, cold or dry air and becomes inflamed. Those who work in conditions which expose them to infections like health care personnel, those who work with irritants like smoke, petrol and pain fumes, perfumes and different sprays are also at risk of developing sinusitis.

Those who smoke a lot and who have a weakened immune system are more exposed to sinusitis and infections than others.

A person who has fever, headaches, facial pain and pressure, colored nasal discharge, and even problems with the vision for more than 10 days must go to the doctor and ask for a treatment because left untreated, sinusitis can cause pneumonia, ear infection, bronchitis and even bone infections.

The doctor will try to see if there is redness and swelling of the nasal passages, tenderness at percussion and swelling in the eye area. With the help of a rhinoscope the doctor will want to see if the sinuses are obstructed or not. Sometimes he might even perform an aspiration to collect samples of the material that blocks the sinus and see what kind of bacteria populates this material.

The therapy consists out of medical treatment and home care remedies. They are meant to clear up the infection, open and drainage the sinuses, and reduce as possible the intensity of the symptoms while reassuring prevention to recurrences of the infection.

At home the patient is recommended to drink warm fluids, to apply wet bandages on the face and to make steam inhalations. The doctor will prescribe the patient oral decongestants, and mucus thinning agents. Besides this anti-biotherapy is needed in order to stop the infection that has caused sinusitis. The doctor might try more types of antibiotics until he finds the right one as he can not know which bacteria has caused your infection. Also, cortisone sprays might be helpful as they improve the drainage of the sinuses. Treatment must be followed at least 10 days otherwise the sinus infection might come back after a few days.

For those in whom the treatment is not effective surgery is the last option. This surgery lasts two or three hours and it consists out of straightening the septum, and opening the sinuses in order to improve the drainage. Hospitalization is needed a day or two and the recovery period might last up to two weeks. Even if you have had surgery this does not mean you can not have sinusitis again, but the advantage is that if you develop it again you will be able to treat it more easily.

The easier thing is to prevent sinusitis, not to treat it. You can prevent sinusitis by giving up smoking, by treating any colds as soon as they install, by cleaning up the nose gently when needed and by drinking a lot of warm liquids.

    Similarly like in the case of adults, skin tests can be performed in children of any age, but we know from experience that the skin of a child up to the 3rd year is very sensitive and reacts strongly to allergens, often in such a way that makes it difficult to make correct diagnosis.

    One of the most common allergen upsetting small children and infants is the food (ingested allergene). It affects the childs skin more seriously than inhaled allergens. It may lead to an incorrect readings of tests performed in small children. The skin tests carried out on older children are much more reliable. A positive skin reaction occurs if a person is allergic to a particular allergen and if the mast cells of the skin contain the appropriate IgE antibody to this substance. During the skin tests, the mast cells of the skin undergo a process of degranulation in which histamine is released. It is the histamine which causes skin changes such as erythema or bumps. The mast cells of a childs skin contain smaller amount of antibodies and this may be the reason why we cannot observe a stronger reaction to the allergen.

    Skin tests performed in an allergic child may turned out to be negative, when the child is administered antihistamines. Different medicines can also slightly influence the results of the tests. That is why it is necessary to know what medicines has been administered to the child by the child before scheduling the skin tests.

    The following medicines should not be taken by the patient during the period preceding the skin tests:

    * first-generation antihistamines (Clemastine, Fenergan, Diphergan, Hydroxyzinum) for 3-4 days
    \r* cetirizine (Zyrtec) - for 3-4 weeks
    \r* loratadin (Claritine) - for 2-3 weeks
    \r* astemizol (Hismanal) for 4-6.weeks
    \r* ketotifen (Zaditen, Pozitan) for 2 weeks,

    There are three main types of skin tests:

    * puncture, scratch test (\”prick test\”, percutaneous).
    \r* intradermal test (intracutaneous),
    \r* patch test (epicutaneous)

    Patch tests are performed by a dermatologists if a contact allergy is suspected or other reactions typical to occupational allergy. The results can be obtained in 48 or 72 hours. Children are diagnosed using scratch tests. Such tests are considerably easy to perform and correlate well to the specific IgE antibodies in serum and to the appropriate inhaled provocative test under the influence of the same allergen. Skin reaction can be observed in 5-10 minutes after placing the allergen onto the surface of the skin.The test is carried out by placing a small amount of the suspected allergy-provoking substance on the skin. Next, the skin is gently scratched through a small drop of the allergen. The scratched place should not bleed. The results of the test can be collected in 15-20 minutes or even earlier if the blisters extend rapidly and form protrusions, which indicates an occurrence of pseudopodia.

    The best place to perform a skin test is the inner part of the forearm or ones back. Children should be diagnosed by performing the tests on their forearm unless there are no visible skin changes. The skin tests can be considered to be done correctly if a positive control test (1 per cent solution of histamine) and a negative control test are carried out. The last one uses saline extra phenol added to the suspension of allergens as a preservative. If the results of the positive control are negative, the tests cannot be regarded reliable. In such a case, the reaction of the skin is inhibited by antihistamine medicines.

    Such tests should be repeated after some time, having carefully examined the methods of the childs treatment within the last period of time. Sometimes, the reaction with the solution of sodium chloride can turn out to be positive. It may happen if the reagent has been contaminated or the child displays some symptoms of dermographism.

    The extent of the reaction is determined by measuring the diameter of the blister using a special transparent scale. This is the most accurate testing method. A control histamine testing will produce a 5-7mm blister. In order to confirm a positive result of the test, the diameter of the allergene-provoked blister should not be smaller than 3mm. All tests carried out without positive and negative control testing are unreliable. Another method of determining the results of tests is marking the examined places with (+) or (-). If the diameter of the blister is at least 5mm, the histamine test result is marked with a triple pluses +++. In this method, which is not recommended, the smallest positive place is labelled with ++, which corresponds to a blister allergic reaction of 3mm. The blisters usually disappear after several minutes or after an hour. In some cases, after 4-6 hours, a delayed reaction may occur.

    During intradermal tests, a small amount (0,05-0,1 ml) of a special allergen solution is injected into the skin. The readings should be made after 15-20 minutes. This test is more painful and it produces more false positive results.

    Skin tests, which are the basic methods of diagnosing allergies, cannot be carried out on patients suffering from severe allergic reactions (e.g. an allergic shock resulted from a contact with allergen mentioned in the case history). In such situations, a safe method of diagnosis consists in isolating characteristic IgE in the blood. In some very rare cases, during performing the skin tests an anaphylactic reaction to an allergen may occur. Although such reactions are usually harmless, it is necessary for an allergist to have an anti-shock medical kit at hand in order to help the patient if necessary.

    The majority of the medical community believes that there are no cures for allergic rhinitis, a condition characterized by sneezing, runny nose, watery eyes and commonly referred to as hay fever.

    Authorities on allergic rhinitis state that the only treatment includes avoiding allergens, taking antihistamines and possibly allergy shots when symptoms are severe or difficult to live with. However, some medical professionals and former sufferers of allergic rhinitis state that there are possible cures for allergic rhinitis, that they themselves have been cured and are free of symptoms.

    But take these comments for what they are — anecdotal, not scientific, evidence.

    Allergic rhinitis can lead to sinusitis, which is a chronic inflammation of the sinuses. Frequent sinus infections may occur in those who suffer from allergic rhinitis. In some cases, surgery is necessary. So, looking for cures for allergic rhinitis is important for those who wish to avoid complications or chronic conditions.

    While some may think of hay fever as only moderately serious, the symptoms can negatively impact the quality of a persons life and the common treatments for allergic rhinitis do not always improve that quality.

    Although antihistamines are not cures for allergic rhinitis, their use for treating the condition is widespread. Antihistamines are not cures for allergic rhinitis because they do not stop the allergic reaction. They may relieve some of the symptoms of allergic rhinitis temporarily. However, there are potential side effects of synthetic antihistamines, which include: dry mouth, throat, eyes and drowsiness.

    Natural antihistamines do not cause these side effects.

    In 2002, a team of scientists at the Tohoku University in Japan compared the antihistamine effects of the mangosteen to a drug used to treat allergic rhinitis and found that mangosteen extracts were potent natural antihistamines and were just as effective as the prescription drug. So, mangosteen could be one of the possible remedies for allergic rhinitis, because it blocks histamine production, but what about inflammation?

    Another team of scientists at the same University found that a component of the hull of the mangosteen was a COX-2 inhibitor. They were not looking for alternative treatments or cures for allergic rhinitis, but simply studying the components of this fruit that was used in traditional Asian medicine.

    COX-2 inhibitors are powerful anti-inflammatories, but synthetic forms of COX-2 inhibitors have caused numerous side effects. They relieve pain, but hamper a compound that helps prevent dangerous blood clots from forming in the arteries.

    As previously mentioned, scientists believe that natural anti-inflammatories or COX-2 inhibitors are safer to the human body than synthetic anti-inflammatories. So, the mangosteen is a natural antihistamine and a natural anti-inflammatory.

    Now you may be wondering if anyone has used it for the treatment of allergic rhinitis and if it could be one of the natural cures for allergic rhinitis. What follows is a brief synopsis of one testimonial relating to allergic rhinitis and mangosteen use.

    A medical doctor holding a Master of Public Health degree from Harvard University reports that he, himself, suffered from allergic rhinitis for a number of years. His symptoms were pounding headaches, nasal congestion and sneezing. Over the years, the allergic rhinitis led to repeated sinus infections requiring treatment with antibiotics.

    The prevalence of enzyme sensitization in the bakeries was 7.8% when all the employees were taken into account and 11.8% in the bakers group. The prevalence coincides with those report edelsewhere in Europe, for example, 516% in the United Kingdom (Cullinan et al 1994, Smith & Smith 1998, Jeffrey et al 1999,Nieuwenhuijsen et al 1999), 9% in The Netherlands (Houba et al 1996), 7.5% in Italy (DeZotti et al 1994), and 19% in Germany (Baur1998a).

    The prevalence of flour sensitization in bakeries was12%, which was on about the same order as elsewhere in Europe in the aforementioned studies (6%24%). The employees sensitized to enzymes belonged to typical groups exposed to flour and enzymes,but a detailed exposure profile was not included in the original study design. Thus further exposure-response assessment was not possible.It was learned that the job tasks of many of the sensitized workers had varied over the years. The study demonstrated that exposure to powdered additives containing enzymes leads to sensitition in Finnish bakers. In the rye crisp factory, where exposure was less on the whole than in the bakeries, sensitization to enzymes was rare. Cellulase was shown to cause sensitization even at moderate exposure levels. There was a significant association between enzyme sensitization and work-related respiratory symptoms.

    However, co-sensitization with flours was common, and the origin of the symptoms was difficult to determine.The animal feed industry is a new area of enzyme application. It was demonstrated that enzymes pose an allergy risk in this industry,too. The risk is smaller than, for example, in the baking industry, but,still, cases of occupational asthma due to enzymes have occurred.There was a correlation between sensitization to enzymes and work-related symptoms, and some of the sensitized workers reported symptoms when exposed to enzymes. One of the enzymes to which sensitization was shown was phytase, which has been developed solely for use in animal feeding. Previously, sensitization to phytase was found in Finnish enzyme production (study II). These were the first reports of allergy caused by this enzyme.\r In the detergent factory, a surprisingly high prevalence (22%) of symptomatic sensitization was found among the process workers.

    In addition to established allergens in the industry, the bacterial proteases, sensitization to new enzymes such as lipase and cellulase was detected. This was the first publication on the allergenity of these enzymes in the detergent industry. Sensitization to bacterial a amylase (Termamyl ) in the industry had been noted since the late 1980s (Sarlo et al 1997). In a later paper from the same multinational detergent company it was also reported that sensitization to lipase had been detected in the early 1990s (Peters et al 2001).

    In addition the present study showed that enzyme allergy is still possible, and it can even occur at a high rate, in the detergent industry, despite the use of encapsulated enzyme preparations that are principally considered nondusty and safe. This conclusion received support recently from the study of Cullinan et al (2000) in the United Kingdom. The prevalence of sensitization and clinical allergy in these two studies was in clear contrast to the prevalences\r reported by large multinational companies (Cathcart et al 1997, Schweigert et al 2000, Peters et al 2001). Obviously there are large variations in industrial hygiene conditions between different plants.

    Small industries may find it economically difficult to meet all the standards of the industrial hygiene programs conducted in large companies, including periodic health checks and continuous monitoring of workplace air by immunoassays (Schweigert et al 2000,Nicholson et al 2001).\r In the enzyme-producing industry, a high rate of sensitization, up to 25%, was noted in subgroups with high levels of exposure.

    The division into subgroups enabled exposure-response (sensitization and respiratory symptoms) calculations, and a statistically significant trend was found for both sensitization and symptoms. A unique feature was the high allergy risk of the laboratory personnel. It turned out that powdered enzymes had been handled rather carelessly, and, clearly, an awareness of the sensitizing properties of the cellulolytic enzymes was lacking.

    In addition to the sensitization of the employees who handled enzymes themselves or worked in the vicinity of enzyme handling, some cases of sensitization were found among people not involved directly in production, for example, among the cleaning and maintenance personnel and also among office personnel with occasional exposure. Maintenance workers may be exposed to the\r highest peak concentrations of enzymes in the workplace. The practice of hiring personnel on a subcontract basis is becoming general, and this practice increases the probability of being exposed to enzymes. The experiences gained in the subcontract plant that spray-dried cellulase confirmed the importance of proper and sufficient information. The employees in the subcontract plant were clearly ignorant about the sensitizing properties of the substances they were handling.
